Ty Montgomery, DNP, ACHPN
Ty Montgomery, DNP, ACHPN, is a Family Nurse Practitioner who has been in practice for over 15 years in Northern Arizona, and in healthcare for over 35 years. After beginning his career as a firefighter/paramedic, he then went into nursing and worked in the Emergency Department and ICU before going back to school to become a Nurse Practitioner. He opened a private family practice in Sedona in 2006 which was then purchased by the local hospital in 2010. Following that he went to work with a national Hospitalist company and has worked extensively over the last 13 years in physical rehabilitation, geriatrics, and hospice medicine. He is now returning to a more traditional office based family practice as well as continuing to follow his patients in the rehabilitation and hospice settings. Dr. Ty holds Master's and Doctoral degrees in nursing, receiving his Doctor of Nursing Practice from Touro University Nevada. He holds national board certifications in both Family Medicine and Palliative Medicine. He relocated to Prescott in 2019 with his family, and is excited to be offering this new primary care model to our community.
